Please have a health examination at the campus you belong to.

The date and time of health examination are divided by the gender, grade, faculty, and student ID number. Please be sure to check the "Health Examination Schedule".

If you need consideration, please contact us in advance. We will respond individually.

Print the“Health Examination Form”in A4 size ,fill in the necessary items ,and bring it with you on the day of the health examination.
(Common to Imadegawa campus and Kyotanabe campus.)
